feat(model-fetch): fetch Kiro models from upstream

- add Kiro ListAvailableModels request planning and headers
- route Kiro model refresh through upstream fetch
- normalize Kiro model payloads and default model metadata
- remove profileArn requirement from ListAvailableModels
