dayuan.jiang 65625ba212 docs: record how the provider refactor plan was carried out 
Steps 0 to 8 are on this branch; the record lists where the implementation
departed from the plan (steps 1 and 2 merged, Azure's placeholder removed in
step 5, a narrower trigger for the automatic model list, the snapshot's
sixteenth shape, line counts) and the checks that were run.
