refactor: extract diagram tool handlers to dedicated hook

- Create useDiagramToolHandlers hook for display_diagram, edit_diagram, append_diagram
- Remove ~300 lines from chat-panel.tsx
- Remove unused stopRef
- Gate debug console.log statements with DEBUG constant
