refactor: 移除 KeyFormDialog 中的 OAuth 认证逻辑

OAuth 流程已由独立的 OAuthAccountDialog 处理，移除 KeyFormDialog 中冗余的
OAuth 表单、授权流程 UI 和状态管理代码。同时清理 provider_oauth.ts 中不再
使用的接口和函数，修复 ProviderOAuthCompleteRequest 重复声明问题。
